All Berkeley Lab employees, visitors, and contractors may use this form to report a Fire Safety Concern. The Fire Safety Concern form is for non-emergency purposes only.

This form is not monitored 24/7. Reports will receive a response by the next business day (Monday through Friday, 8:00 AM to 5:00 PM). For immediate assistance regarding non-emergencies, please contact the Site Operations Center at 510-486-6999.
Completing the Form
Provide as much information as possible to allow the Fire Inspector to locate and address the concern. You may enter your contact information if you would like to be contacted during the investigation and resolution process, or you may choose to remain anonymous.
After submitting the form, you will receive a confirmation message on the screen.
Post-Submission
Submitted reports are reviewed by the LBNL Fire Marshal’s office. A Fire Inspector is assigned to investigate and will take all necessary action to resolve the concern in a timely manner.
If contact information is provided, the Fire Inspector may contact you for more information. The Fire Inspector will take appropriate action to resolve the reported Fire Safety Concern. If you chose to be notified upon resolution, the Fire Inspector, or a representative, will notify you once the concern has been resolved.
