LBNL currently issues uncleared HSPD-12 credentials on a limited basis. If you require a credential please have your supervisor contact us at HSPD12@lbl.gov.
See below for the uncleared HSPD-12 credential workflow:

- Initiation: The applicant’s supervisor is to reach out directly to HSPD12@lbl.gov with the applicant’s employee ID and short justification as to why the applicant requires an uncleared HSPD-12 credential.
- Internal Justification Form: LBNL Personnel Security (PerSec) initiates an HSPD-12 Request Justification Form to be completed by the applicant’s supervisor via HelloSign. This form is then signed by the supervisor and SES Department Head of Site Security. The justification must have a detailed description as to why access is specifically required to support Contract 31. Note: A request cannot be made for unspecified work or for convenience purposes.
- Position Designation Record (PDR): LBNL PerSec reaches out to the applicant’s supervisor and HR partner to generate a PDR via the Position Designation Tool (PDT). The results generated determine what level of investigation should be conducted for the applicant’s position.
- SC 206.2 & OF 306: LBNL PerSec reaches out to the applicant to complete the SC 206.2 & OF 306 forms and deems assistance as needed. LBNL PerSec then collects signatures from the Department Head of Site Security and Federal Contracting Officer on the SC 206.2 form. The applicant is then sponsored in USAccess.
- Enrollment: LBNL PerSec reaches out to the applicant with detailed instructions to complete USAccess enrollment. Enrollment requires an in-person 30 minute meeting to take a photo, fingerprints, and present ID documents. To review a complete list of acceptable forms of identity, please visit GSA.gov/Federal-Credentialing-Services. Enrollment can be completed at LBNL or any shared USAccess site.
- eAPP Questionnaire: LBNL PerSec initiates an eAPP questionnaire to be completed online by the applicant. The questionnaire required (SF-85, SF-85P, SF-86) depends on the level of investigation dictated by the applicant’s PDR.
- Sent to DOE: The applicant’s case is sent to the Office of Science Cognizant Personnel Security Office (CPSO) for investigation and adjudication. The timeframe for completion depends on the level of investigation ordered. The “Investigation Timelines” table below provides an estimate for the length of an investigation.
- Credential Activation & Pick Up: LBNL PerSec reaches out to the applicant to schedule an HSPD-12 credential activation once we receive the credential. This action is completed onsite at building 65.
Important Notes:
- Physical credentials generally expire five years from the printed date on the credential.
- Digital certificates on the credential expire three years from the printed date on the credential. The card holder receives email notifications when they are close to expiration. If the certificates expire, the credential is terminated.
- HSPD-12 credentials are government property. You are required to surrender your credential upon termination of your employment with LBNL or by management request.
